Agrahara claim form has been updated by the National Insurance Trust Fund. Future claims should be made through the relevant form. That claim form can be downloaded by visiting the website, www.nitf.lk (of the National Insurance Trust Fund). From 29.02.2024 only this form should be used and it has been informed that the claims made through the previous form will not be accepted.

When applying for Agrahara insurance claims, visit the website of the National Insurance Trust Fund and submit the duly completed claim form along with the certified copies of all the documents related to your requirement and the originals of the relevant bills to the Ministry without delay. The Ministry is not responsible for rejections due to the shortcomings in documents.
